Our CCP Program was developed to accurately predict and effectively provide for the resource needs of our patients with chronic medical conditions. Based on the completion of a comprehensive assessment at the patient admission visit, additional resources are identified which may be appropriate to meet the patient's needs, including nursing services, rehabilitation services, and telehealth monitoring. The overall goal of CCP is to provide comprehensive care through an interdisciplinary collaboration of care.
